POSITION SUMMARY
The Transition of Care Nurse provides consultative and coordinating functions in the transitioning of patients from the acute care facility to home or alternative care sites. This position works in conjunction with the Nursing and Medical/Case Management departments of the hospital and at the direction of the management of AtlantiCare LIFE Connection. The Transition of Care Nurse also is responsible for the coordination of care of all AtlantiCare LIFE Connection participants currently residing in an acute care setting.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.
QUALIFICATIONS
EDUCATION: Graduate of an accredited school of Nursing required. Bachelor Degree in Nursing preferred.
LICENSE: Current Licensure as a Registered Nurse in the State of New Jersey. Valid New Jersey Driver's license, Proof of Registration and Insurance required. BLSHCP required.
EXPERIENCE: Minimum 1 year with working with the frail or elderly required. Minimum 1 year experience as a Home Health or Long Term Care Nurse or equivalent experience required, 2 years of medical/surgical experience required. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
